Please read and share 🙏👇🏾 ★Exploring Direct Access in Physiotherapy- Why Doctor of Physiotherapy program? 🔊★ I recently came across a letter from the Association of Provosts of Colleges of Medicine in Nigeria expressing concerns about the proposed Doctor of Physiotherapy program. The main concern raised was related to patient safety, with questions being raised about the potential risks associated with direct access to physiotherapy. In this thread, I will address this concern. It's important to highlight right away that the safety of patients is our top priority, just like it is for any healthcare professional. As physiotherapists, we are trained to identify "red flags" that indicate situations where patients may require more specialized care outside of our area of expertise. Direct access to physiotherapy isn't meant to replace the traditional referral system between doctors and physiotherapists, but rather to provide an additional entry point that can help alleviate some of the burden on the primary healthcare system. It's a valid point raised by a New York Times article that physiotherapists are not trained to diagnose back tumors. Diagnosing such conditions requires advanced imaging and biopsies, which are typically performed by medical doctors. However, physiotherapists can still play a crucial role in screening for conditions like malignancies and identifying red flags that necessitate a referral. For example, individuals who complain of continuous back pain without any mechanical cause, pain at night, have had cancer in the past, are over 50 years old, experience general symptoms such as weight loss, fatigue, and low-grade fever, or have not seen any improvement with conservative treatment require thorough evaluation. Although not every patient with these symptoms may have cancer, it is important to refer them to a specialist for further examination. It is crucial to differentiate between diagnosing pathology and screening for pathology. Irrespective of how patients come into our system, either directly or through referral, we always screen for non-mechanical conditions. It is worth noting that the direct-access physiotherapy system has demonstrated its usefulness by bringing down healthcare costs, reducing the utilization of resources such as imaging, enhancing patient satisfaction, and improving health outcomes. It is possible that you may be wondering why we conduct full assessments even for patients who are referred by doctors and how a doctoral degree strengthens our capabilities. Research conducted in 2012, involving 78 patients who were referred by doctors for musculoskeletal pain to physiotherapy, revealed that all of them were referred back to doctors due to the physiotherapists suspecting sinister conditions during assessments. This is not to undermine the doctors, but to highlight the common occurrence of missed diagnoses in primary care, which is often attributed to high workloads. Demystifying Physiotherapy: ▶︎Bridging Gaps in Medical Understanding & Collaboration◀︎ 🙏 repost: This is the year 2023, and as a medical practitioner, there's no excuse for you not to understand the roles of all the players in the multidisciplinary team (MDT). I will try to explain what you need to know about Physiotherapy, what we do, and why we are different from any other member of the MDT. Physiotherapy (PT) is a profession whose main aim is to minimize impairments and encourage a return to full functionality. PT manages a person rather than their condition. However, for us to manage an individual, we need to completely understand their impairments from the cellular and organ-system level. We also need to understand their desires, thoughts, beliefs, etc. At the end, we provide them with a personalized treatment plan. Let's consider this case: Mr A, a 56-year-old engineer with pain in his right shoulder, just walked into my PT clinic. Mr A: With the understanding that his right shoulder pain could be musculoskeletal (muscle, tendon, ligament, nerve, bone, joint, joint capsule, Bursa) and non-musculoskeletal (systemic inflammation, Cancer (Pancoast, thyroid, bone), referred pain from the Visceral tissues (Gall bladder, Liver, lungs), etc., causes. I need to complete a full assessment for Mr A. This will include a full subjective assessment to understand how his pain came about, how severe it is, and the overall nature of it. Also, I'm interested in how it limits him at work, affects his sleep, etc. Following this, I’d check the shoulder itself to identify any deficit in strength, movement, etc. I'll also complete abdominal screening, particularly the right upper abdominal quadrant for him. From my understanding of pathologies and how they present, I can make a decision on what Mr A needs to do. So let's say I discovered he had pain on his other shoulder sometimes + pelvis pain, headache, and pain on the scalp when he combs his hair, and his pain is often worse in the morning. Based on this, I make a diagnosis of Polymyalgia rheumatica. I can REFER him to the LABORATORY for blood tests (CRP, ESR, FBC) while also, based on my intensive study of Pharmacology and my understanding of the important roles of PHARMACISTS, I will REFER Mr A to start a course of oral corticosteroids. As a PT, I will see Mr A more than any other team, so I'm in the best position to OBSERVE the drug response for a few weeks. If he doesn't get better as expected, I will REFER him to the DOCTOR or specifically a rheumatologist. Also, I can start him on simple functional exercises to get him back to work. This is an example of how it should be if everyone understands their roles and responsibilities as a member of the team. It also highlights why physio students have to master anatomy, physiology, biochemistry, pharmacology, pathology, sociology, psychology, and physiotherapy specific skills. Notice I'm not offering Mr A MASSAGE as always depicted online? That's because the PT profession has evolved over the years. Even if I offer Mr A a THERAPEUTIC massage, it doesn't push aside my REASIONING process up to that point. NOTE: This is just a sample case for a musculoskeletal physiotherapist. We have neurological and cardiopulmonary physios who have devised their specific management strategies too over the years. A physiotherapist would not typically treat two patients with the same medical conditions the same way because our treatments are personalized. Can you see how complicated physiotherapy is? So there's absolutely no reason to want to take up more roles. Millions of people that need rehabilitation in Africa don't have access to it according to the WHO. One of the reasons is the poorly utilized and ineffective referral system. Just imagine a 40-year mother of 3 who couldn't return to her job after suffering a stroke. When she could have been referred to be rehabilitated back to her society to take care of her children. ⬇️
