This is the whole design question for anyone building something they actually care about. We serve family caregivers taking care of a parent with dementia. That is pillar one, and it is why the company exists. It doesn't pay for itself. So we built pillar two: the provider, who gets paid for reviewing what the caregiver reports. Pillar one is why we exist. Pillar two is why we can. The caregiver never sees a bill. Turn those around and you get a documentation tool with a caregiver attached. Nobody needs that. You don't create to make money. You make money so the creating survives contact with reality.

An ICP is a person" beats "SMBs." But name the job, not just the person. User, chooser, payer are often three different people. Design for one. Sell to another. Bill the third. Otherwise you've shrunk the vague answer, not fixed it. At Atendacare we design for the caregiver , and that caregiver has a narroer definition. We sell to the provider. The payer {in our case Medicare), we comply with.

Three psychiatrists. Same training, same rubric. They could not agree on what makes an AI response to a mental health crisis safe. A new Stanford study had three board-certified psychiatrists rate 360 AI responses across eight clinical dimensions. Reliability was poor on every one. On the highest-stakes questions, suicide and self-harm, they disagreed the most. One rated 92% of responses inadequate on whether the AI disclosed it was not a clinician. Another rated those same responses fine. The disagreement was not sloppiness. Each psychiatrist held a coherent, defensible clinical philosophy: one led with safety, one with engagement, one with cultural context. All three are right. Average them and you get a number that represents no clinician at all. The conclusion: a general-purpose AI, asked to be everything to everyone in mental health, has no stable ground to stand on. This is the problem AtendaCare was built to solve. AtendaCare is a purpose-built clinical companion for the family caregivers of people living with dementia. The population is known and the clinical framework is declared in advance. The scope is narrow by design. It does not make the clinical call. It captures what happens over the month and delivers a structured summary to the prescribing physician, who reviews and attests. The clinician carries the judgment. The platform carries the watch. The Stanford authors, in their own recommendations, draw the line exactly here: general AI cannot be made reliably safe for mental health, but purpose-built tools with a known context and a declared philosophy can. UnitedHealth's earnings call last week highlighted a 35% reduction in skilled nursing facility admissions through home-based care. Real number. Real shift. But there is something nobody on the call said out loud. A Thread #Caregiving

$446 billion. That is the value of unpaid care provided by family caregivers to people living with dementia in 2025. 19.6 billion hours of work. 12.7 million caregivers. No paycheck. No benefits. No 401(k). If America's family dementia caregivers were a single workforce, they would outnumber Walmart, Amazon, and the U.S. military combined. The report says 7.4 million Americans 65 and older have Alzheimer's today. By 2050, that climbs to 13.8 million. The caregiving load doubles with it. These caregivers are not abstractions. Two-thirds are women. About a quarter are raising children at the same time. Forty-one percent earn less than $50,000 a year. They sleep less. They get sick more often. They face depression at higher rates than people their age who are not caregiving. I know because I am one of them. The largest health workforce in America has had almost no real support. Build it and they will come is a movie, not a business plan. I spent decades in real estate development. One rule kept me out of trouble: never build a medical center hoping you’d find a tenant. Never build a house hoping someone would want to buy it. You find the need first. Then you build. I brought that same discipline to AtendaCare. Before we wrote a single line of code, we talked to caregivers. We listened to what was breaking them down. We identified the physician who would prescribe the platform, the family caregiver who would use it at 2am, the Medicare billing pathway that would pay for it. We architected around real people with real problems, in many cases with their direct input. A lot of founders do the opposite. They build something beautiful, then go looking for the customer who needs it. Some get lucky. Most don’t. The difference is not just strategy. It is respect. When you design something with your customer in mind from day one, you’re saying: your problem matters more than my idea. That’s how AtendaCare was built. The architecture came after the listening. If you’re a founder, ask yourself this before you write another line of code: do you have a committed customer yet, or just a committed idea? Those are very different things. What is a moat? Most people think of it as a barrier. Dig a ditch around your castle, fill it with water, and the enemy can’t get in. Simple enough. But a moat is only as good as the day it was dug. A static moat is just a puddle. The enemy studies it, measures it, and eventually figures out how to cross it. Time is on their side, not yours. A real moat grows. When we started building AtendaCare, I heard the same thing every founder hears. “Someone bigger will just copy you.” And honestly? They’re not wrong about the copying part. Anyone with resources and time can replicate what exists today. What they can’t replicate is what exists tomorrow. Every conversation a caregiver has with Atenda makes the platform smarter. Every voice journal stored in the Memory Vault makes that caregiver harder to move somewhere else. Every behavioral pattern we learn makes the next conversation more accurate than the last. The knowledge base we built is a living thing. It grows every single day we operate. So yes. A competitor could start building our castle today. By the time they finish, the moat they face will not be the one that exists right now. It will be two years deeper, two years wider, and two years harder to cross. They are not chasing a target.
