First is patient selection - I'm a big believer in allowing biology to declare itself. BR002 reminds us that even that may not be enough. Regarding RT - modality probably not as important as dose, volume, and coverage.

@drymtn IF there is any role for chemoRT in gastric, it's neoadjuvant. I'm not sure you can reliably identify an at-risk target post-operatively. And the efficacy will be limited by relative hypoxia in the field.

@KrishanJethwa I treat inguinals for any anal canal involvement. This is a disease for which we maximize local control. I might reconsider for patients at elevated risk for toxicity (e.g. obesity) if minimal borderline involvement and cN0.
