Sudden monocular, painless, complete vision loss in an elderly patient. POCUS reveals a retrobulbar hyperechoic focus (“spot sign”) from an embolus causing a central retinal artery occlusion (CRAO), an ocular emergency. Consult Optho immediately, globe massage, IOP reduction.

PYLORIC STENOSIS: find gallbladder w linear probe > identify pylorus longitudinally (posteromedial to gallbladder). Remember “Pi 3.14” for “Pi”loric muscle wall thickness > 3mm and length > 14mm. With pt laying R side down, pylorus is not seen to open. Rotate 90° for target sign.

Saddle pulmonary embolism caught on TTE using a modified parasternal short-axis base view to assess the pulmonary valve and the pulmonary artery. To see the bifurcation of the pulmonary artery, it may be necessary to move to a lower intercostal space.

Eosinophilic myocarditis, a hypersensitivity response that may be induced by medications (e.g. antibiotics, pressors, diuretics), neoplasia, parasitic infections, or autoimmune disorders (e.g. Churg–Strauss syndrome, Loffler disease). Absolute eosinophil count here was > 70,000.
