@DrJMarine We look to experts from the CDC, etc to provide guidance on all sorts of public health issues. It’s unfortunate there wasn’t full transparency about the effectiveness of all the recommendations, because people now lack trust in them.

@drjohnm I had to do a peer to peer once for a patient referred from the CHF service for ICD with EF 30%, NYHA Ii, no ischemia, optimal GDMT > 3 months. Everything plainly stated in the chart. There needs to be some penalty for these kinds of denials. No downside for insurance to deny.

@AlbertLinMD1 Clinically, if you have a patient with truly concerning bleeding history & high risk for stroke, the patient may considering OAC or nothing. LAAO seems to be better than no therapy & does have less long term bleeding if the procedural bleeding can be mitigated.

@drjohnm The main selling point of LAAO has been to provide stroke risk reduction in patients who can’t tolerate OAC. The trial does show a stroke reduction effect of LAAO, but also shows that these patients can tolerate DOAC. Many referrals are for softer bleeding issues that impact QOL

@KurtSupeCPA People also misunderstand primary & secondary beneficiaries on pay on death benefits like life insurance. It’s not like a QTIP. Primary gets the entire benefit & they can then do whatever they choose with the funds.

@ProfDConnelly @ncurzen @mcfinlay AF ablation is not a guaranteed cure. It is the most effective rhythm control therapy available. Patients most likely to benefit from PVI are those early in the disease process with well controlled risk factors (normal BMI, avoid Etoh, nml BP, A1c, treated OSA, etc).

The recurrence rates for symptomatic AF surely demand a rethink about the application of ablation to this population? Isn’t it time to find out who is likely to benefit & who isn’t? Do the patients with recurrence get advised to have it again?
