🚨New in #EHJCVP 💧Kidney function and acid-base status as gatekeepers of diuretic response in chronic heart failure💊 This pre-specified post-hoc analysis of the DEA-HF randomised crossover trial compared three regimens in congestion-refractory HF: IV furosemide alone, furosemide plus oral metolazone, and furosemide plus IV acetazolamide. With eGFR above 30, adding metolazone produced greater natriuresis than furosemide alone or furosemide plus acetazolamide. Below eGFR 30, neither add-on gave extra benefit. Higher serum bicarbonate predicted better natriuretic and diuretic response and more benefit from metolazone. 🛟Safety was comparable across the eGFR range. New in #EHJCVP 💊SSRIs are commonly prescribed in patients with coronary artery disease, but what happens after PCI during dual antiplatelet therapy? 🫀 In this large real-world #TriNetX analysis of nearly 30,000 patients, concomitant SSRI use was associated with a higher risk of major bleeding🩸, without an increase in ischaemic events. The study benefits from a large international cohort and propensity-score matching, but its retrospective design leaves room for residual confounding, while prescription data cannot fully capture treatment adherence and persistence of all treatments or bleeding severity.
