Interesting from @JAMA_current: In a huge study of >2 million prescriptions, 32% were rejected by insurance. Of those rejected, only 38.6% eventually got the original med, 13% got something in the same class, and nearly HALF (48.4%) got nothing equivalent within 90 days. How many of these patients who needed a therapy got nothing at all? jamanetwork.com/journals/jama/… This hits home hard in peds pulmonology. Plenty of asthma meds are “approved” for kids as generic dry powder inhalers (DPIs). The problem? A lot of little lungs simply can’t generate the strong, fast inhale needed to get the medicine where it needs to go. Poor delivery → poor control → more ED visits → kids labeled “difficult to control asthma” who then get sent to specialists… when the real issue was the device they were given. Even “cheaper” generics create barriers when formularies don’t account for age, technique, or what actually works in real life. How do we fix this? #Asthma #PediatricAsthma #TinyLungs #HealthPolicy

Pediatric pulmonologist @PedLungDoc recently donated over 70 bags of medical necessities for Baltimore City students with #asthma. The bags included an albuterol inhaler, reusable spacers, and educational materials for public schools. The bags will cover over 40 health suites in different public schools. A new law in Maryland requires public schools (and allows private schools) to have emergency albuterol on hand for respiratory emergencies. This donation ensures that the schools will be able to properly administer the emergency medication to students with asthma. #asthmaAwareness #JohnsHopkins #KidsHealth

But none of this is true: outside of annual flu vaccines it’s closer to 30 immunizations, all or most tested in placebo-controlled trials. We’ve gone from immunizing kids vs 8 diseases when I was a resident in the 1980s, to 16, including 4 for bacterial meningitis, 2 for cancer
