This is really fascinating since we often think of ketamine as being hemodynamically neutral, however, ketamine itself is a direct myocardial depressant. This is usually offset by the adrenaline release it stimulates. -Overall, when compared to etomidate, there was a strong trend towards ketamine being associated with peri-intubation hypotension (OR 1.38, 0.99-1.94) -Among prehospital studies there was a similar albeit pretty weak trend (1.76, 0.72-4.33) -Among RCTs only another pretty weak trend (OR 1.43, 0.37-5.49) However, it's worth noting a few things: -The number of RCT's is relatively low on the topic. The RSI trial, which is underway now, will probably give us a much better answer. -A lot of this was observational data, and based on other subanalyses it appears that this is much more pronounced among patients with pre-existing signs of hemodynamic instability (probably catecholamine depleted) before induction and less so among those without pre-existing hemodynamic instability.
