@orthotraumamd In my experience the least invasive option is usually the best option. That's why I would always opt for MIS procedures to only get rid of the ulcers instead of big heroic reconstructions, if a foot is stable and plantigrade.

@orthobullets It depends on the foot: If the foot is plantigrade and stable and if it's possible to fit it into an orthopedic boot, I'd do a bone resection only to get the ulcer to heal. (MIS if possible) If the foot is unstable and not plantigrade, I'd do the full reconstruction.

Probably try to avoid this if your colleague wasn't successful there.

@solimanibra92 @InvictaOrtho @Gnomelover1970 @anklefootmd @bonesmith_ @centerofhip @Drlyndonmason @eric_novack @neilorpen @TJimulia I suspect 2 problems here: - the fibula is malreduced (too short) - the syndesmotic screw is running behind the tibia thus pushing the joint the wrong way I'd get a CT to better understand what's going on and then revise immediately if there are no wound problems at the moment.

@SurgeryBro A patient during my residency lost most of his lower leg muscles due to a dissection of the popliteal artery. Of course he had pulses when he first came in. When he developed signs of ischemia, it was too late. I wish we would have called vascular surgery.
