In the 90s, i was on a commission in Arlington Texas to deal with a large budget shortfall in the school district. Voters had shut down a tax increase. We solved the “crisis” almost entirely by cutting admin staff. And it is worse today.

one of the more interesting responses to the newsletter came from an anon urologist in Texas walking me through how they get screwed in billing that favors hospitals high level - They do ultrasound biopsy - New tech comes out (MRI fusion) but CPT doesn't account for it. They give the patient and Advanced Beneficiary Notice and the patient pays extra for it. - New CPT accounts for it, but it's not enough. Also means the practice can't do an Advanced Beneficiary notice. The practice would now lose money doing this - Practice then has to refer out somewhere that gets paid a facility fee, which costs way more even though it doesn't need to be done there. This feels like one of the core flaws of CMS and the CPT coding system that uses “how much did that cost” as the baseline for figuring out reimbursement. Of course if you use that methodology you will end up with people shifting care to higher cost settings. Walk through of the economics they gave me below

As a personal example. We had Kaiser, my wife had to get an ultrasound for some female issues. We went in and asked how much it would cost. They told us, with insurance, $2800 because we had not met the deductible threshold yet. While I was waiting, one of the secretaries told us that if we went to another Kaiser facility and asked to pay cash it would be at least $2000 cheaper. We canceled the appointment right there and went to the other facility in Irvine, CA. We just said that we didn't have insurance and they charged us $290 minus $50 cash discount, so $240 for the same scan! This is the scam, and it has happened to us 3 times in the last six months, so we just started paying cash all the time and canceled our insurance. We now get seen same day, any doctor we want and save up to 10x the price. I am getting a CAT scan next week for $350 cash. kskimagingcenter.com/exam_pricing
