I work in the industry and genuinely know of a reader who missed the 'Glaswegian accent of the MC' part until the last hour of their read. I also dealt with a read where the killer had an accent that you wouldn't know on paper but gave the plot away in the 1st chapter in audio.

A fun thing for an author to do would be to have the last line of the book be something like “he said, in his Jamaican accent, which he’d had this whole time.” Make the audiobook narrator have to start the whole thing over.
