physicians, extenders(ie np and PAs) and nurses have become the face of healthcare’s failures because we are the only human interaction patients ever see. Most denials, delays, and restrictions are driven by opaque insurance rules, hospital policies, and government regulations that patients never encounter directly. You never see who wrote the rule or why it exists. You just experience that your doctor or nurse cannot do what feels medically necessary. That disconnect is exactly why patients feel angry and clinicians feel trapped, and why trust keeps eroding on both sides.

The frustration is real, but the target is wrong. Primary Care Physicians did not invent gatekeeping, your insurance company did. The referral requirement exists because insurance companies want utilization control and prior authorization leverage. Primary Care became a choke
