🫀 CardioNugget™: Why is the Nyquist Limit = PRF/2? Think of a clock. 🕒 Imagine you're taking photos of a clock's second hand. Scenario 1: Many photos per revolution → You can clearly tell it's moving clockwise. Scenario 2: Only 2 photos per revolution → You only see 12 → 6 → 12 → 6... You can't tell whether the hand moved clockwise, counterclockwise, or completed multiple revolutions between photos. Scenario 3: Fewer than 2 photos per revolution → The hand appears to move backward even though it isn't. This is aliasing. 💡 Ultrasound works the same way. Each transmitted pulse is one "photo." PRF (Pulse Repetition Frequency) = number of photos (samples) taken each second. To identify a wave correctly, you need at least 2 samples per cycle. 🚨 Your LDL came back normal. Your cardiologist smiled. And you still had a heart attack. That happens because LDL measures cholesterol weight, not particle count. It can look perfectly fine while hundreds of deadly particles silently invade your artery walls. The test that actually catches this is called ApoB. And most doctors never order it. 🔬 Here is what the science actually says. Every atherogenic particle in your blood carries exactly one ApoB molecule. One LDL particle. One ApoB. One VLDL particle. One ApoB. One IDL particle. One ApoB. One Lp(a) particle. One ApoB. That means ApoB is a direct particle count. Not an estimate. Not a cargo weight. A headcount of every particle capable of penetrating your artery wall and starting a plaque. LDL cholesterol measures how much cholesterol is stuffed inside those particles. You can have a low number with enormous particles carrying heavy cholesterol loads. You can have a normal number with hundreds of small dense particles quietly embedding themselves into your coronary arteries. That second scenario kills people who think they are fine. 💓 The data on this is not new and not subtle. ✅ Mendelian randomization studies across hundreds of thousands of patients confirm ApoB as the causal driver of atherosclerotic cardiovascular disease, not LDL cholesterol. ✅ EPIC-Norfolk: elevated ApoB predicted cardiovascular events more accurately than LDL in prospective cohort data. ✅ The 2026 ACC/AHA lipid guidelines now formally endorse ApoB measurement as a risk-refining tool, particularly when LDL gives discordant or falsely reassuring results. ⚠️ The discordance problem is real and it is common. Patients with metabolic syndrome, insulin resistance, type 2 diabetes, or obesity routinely carry high ApoB with normal or even low LDL. Their standard lipid panel looks acceptable. Their arteries are not acceptable. 🩺 I am a board-certified cardiologist. I see this pattern repeatedly. A patient walks in with an LDL of 95 mg/dL. Looks treated. Looks controlled. Their ApoB comes back at 115 mg/dg. That is a patient with residual risk their previous doctors never quantified because they never looked. A patient who gets ApoB measured, sees the discordance, and works with their cardiologist to drive ApoB below 80 mg/dL can reduce their cardiovascular event risk in a way that targeting LDL alone never achieves. That is the difference between a false sense of security and an actual strategy. 🫀 Plaque does not build because of cholesterol weight. Plaque builds because particles enter the artery wall. Particle traffic is what matters. ApoB counts the traffic. LDL does not. The question is no longer whether ApoB is useful. The question is why your doctor has not ordered it yet. ❤️ Bottom line: LDL is not the full picture. It never was. ApoB counts every atherogenic particle with one molecule per particle. It catches the risk LDL misses. It is now guideline-supported in 2026. Ask your doctor for it by name. If your ApoB is above 80 mg/dL and you have risk factors, that number needs to come down. A normal LDL with a high ApoB is not a green light. It is a missed diagnosis.
