Off proximal suture line. If pt can tolerate redo open case then should be a pretty straight forward case.

Now with root aneurysm and arch aneurysm. Unclear exact size but appears significantly enlarged. pseudoaneurysm off root or descending. Unclear exactly where with only 1 image. Recommend redo root possible arch.

@MustafaAhmedMD I think with a good extended Myectomy the SAM and MR would be significantly improved. The AV should be assessed intra op. If significant then I would do Myectomy AVR or AV repair. Great case.
