@AlecMuffett TCP/IP has has VPN (tunnels) as part of the protocol for decades. IIRC, it was shipped in Solaris 7 (1998) and probably in pretty much all operating system TCP/IP stacks since 2000. It was essential for tunneling IPv6 over IPv4 as an early use case.

A few hours offering PSA tests in Action Market today. The weather was terrible, but our phlebotomist was continuously busy. The really good thing about this drop-in session was the majority of attendees were high risk - usually these groups are the most difficult to reach.

@chriseden99 Spoken with a few patients who've been investigated. In all cases, it's failure of Testicles to respond to LH. In one case, there was a very brief T recovery, quickly followed by T collapse. We already knew ADT damages seminiferous tubules, but maybe also a risk for Leydig cells?

An andrology/endocrinology question: In patients who cease ADT, up to half fail to recover Testosterone to an adequate level, or in a few cases, at all. Where/what is the failure in the HPG-Axis which causes this? What causes the typical 9 month delay when full recovery happens?

@VincentGnanapr3 At age 56, I asked for a PSA test, which was 58. No symptoms. Diagnosed T3aN0M0, but high risk of micro-mets. Probably close to going incurable, but was caught just in time. 8 years on, no ED, no incontinence, PSA undetectable. No regrets, very grateful it was caught in time.

@NHSEngland I don't understand. 5 SABR fractions has been a standard of care in England for #prostatecancer since PACE-B trial reported ~2 years ago. Many men in my support group have had this treatment on the NHS over the last 18 months, probably the most popular choice. What's new today?

@TheFocalTC The article says the data is based on those diagnosed before MRI was used as part of the diagnostic pathway. Such old diagnosis methods are not representative of what happens today.

@TheFocalTC A pet peeve is that #prostatecamcer treatments which are not done locally are usually not mentioned to the patient. That often includes focal, brachytherapy, SABR (PACE-B or salvage), etc.

@CuteAdorableCat Cats have a very powerful sense of smell. They can detect more different compounds than dogs, particularly pheromones, but they can't smell at quite the low level dogs can. The cat is picking up hormonal changes, which are probably very similar in pregnant cats. Dogs can too.

We took the Man Van to Brinks UK for a Testing Day, supported by two fantastic phlebotomists from Heim. Together, they did 20 PSA tests, helping raise awareness of prostate cancer. Thank you to everyone at Brinks UK for the warm welcome and to our volunteers for making it happen.
