Yes, this is not an easy problem to solve, especially with the native macOS stack. This is one area where web technology has an advantage over SwiftUI and AppKit. However, it is not impossible. I spent two months building my own Markdown renderer using TextKit for exactly this reason. See how Vinaa handles a long thread here. It could go even faster, but the scroll wheel couldn't keep up. As far as I know, Vinaa is the only Mac app built with 100% native technology capable of doing this. Other apps with similar performance use a web-based Markdown renderer inside SwiftUI.
